Loss of Verification

Step 1

Go to the Verified Burn Center list and select arrow on the Action bar.

Step 2

Make sure the number of records is correct.

Step 3

Look for burn centers that have lost their verification or it has expired. Click on View.

Step 4

On the purchase details tab enter the termination date, select the reason and save. If they had a site visit choose non-verification or non-verification pending review; If they had a visit scheduled and cancelled choose cancelled or if the verification lapsed choose expired.

Step 5

Make sure that the Verified For is blank
