Consultation Determination Letter

Lastly, the determination letter is sent to the burn center. If the recommendation is to pursue verification and there are no comments or CDs, the Verification Consultant can send out the letter to the burn center using this template.

If there the recommendation is to defer verification and/or if there are comments, the Verification Coordinator composes the determination letter using this template.

When comments are provided in the discussion section, the Verification Coordinator edits and inserts the comments into the determination letter. The comments should be edited to be grammatically correct and formal.

For deferred determination letters or otherwise complicated determinations, the letter is to be sent to the VRC for edits.

Once approved, the letter and site report is to be sent to the BC Medical Director and Verification Coordinator. CC the site reviewers, and the VRC Chair.
