Entering Key Findings for Consultation

During and after the site visit, site reviewers are expected to enter their findings and notes into BCQP. Site reviewers are expected to comment on the followings:

• Overall recommended determination

  • For consultation, the options are to recommend verification site visit or to recommend deferring the site visit.

• Comments on previous criterion deficiencies (if applicable)

• Comments on previous Areas for Improvement (if applicable)

• Criterion Deficiencies (if applicable)

• Strengths

• Areas for Improvement

• Case Review

  • Identifying Quality Issues

  • Commenting on PI/Loop Closure

After this is complete, the site reviewer submits the key findings.
