Patients Zip/State/City

Question: There is a patient who is homeless and when referring to the data dictionary for home state and city I am unsure how to document this in BCQP.

Answer: For the “Patient's zip code” enter n/a. Then for “alternate home residence” enter “Homeless”. Address Street 1 & 2 enter “Not known/Not Recorded”. For the “State” enter whichever state they're in or came from. For the city, if it's documented where they usually sleep, shelter, streets, etc.. Then you can put a city, otherwise select “Not known/Not recorded”. At the bottom of the demographics page in the “Living Situation” section enter “Homeless”.
