Application Fellowship
The burn center’s Account Administrator is the only individual who can apply for verification. They must log into members.ameriburn.org to complete and submit the verification application.
You can find a link to the application on the Fellowship Verification page on the ABA website.
You can also find the application when are on your organization’s page in the panel to the left. Scroll down to Verification and click the arrow. Click on Verification Application.
Steps to Apply for Fellowship Verification
Before you start the application, you will need to upload an Application Signature Page. A sample is available to download.
Choose “Purchase for X Organization”
Once on “Certification Application Forms” click on Site Visit Verification Application, then Complete Application Form - click on Start Application.
Enter information into blank fields on application. If having your site visit sperate from your burn center reverification site visit make sure to put blackout dates.
Rescheduling a site visit will result in a $1000 rescheduling fee.
Click Submit & Pay
Select Credit Card or Bill Me and complete the required information.
Then click Submit Your Order.
Once the application is approved by the Verification Coordinator, you will receive a notification that your burn center’s Pre-Review Questionnaire (PRQ) is ready.
