Reintegration Support

Method/Practice: The center felt that their most critically ill inpatients often have unique and more intense needs for emotional, psychological and medical when they transition to the outpatient setting. The institution created in 2019 a Critical Illness Recovery Center which is open to all patients with an ICU length of stay >4 days who had anyone of the following: respiratory failure requiring mechanical ventilation, sepsis or delirium. This clinic is staffed by a pharmacist, a respiratory therapist, a physical therapist, and occupational therapist, a speech language pathologist, ad dietician, a social worker and physiatrist and a MD/APP. The appointment should be made within 2 weeks of discharge. Family members and loved ones are encouraged to attend the clinic appointment with the patient. The visit often last 2-3 hours.
This program may be scalable to most burn centers if, as it is done in this hospital, the program is done in conjunction with other ICUs - medical, trauma, surgical, cardiac.
