Completing the Application
Before you start the application, you will need to upload an Application Signature Page. A sample is available to download.
Choose “Purchase for X Organization”

Once on on “Certification Application Forms” click on Site Visit Verification Application, then Complete Application Form - click on Start Application.

Enter information into blank fields on application.

Please ensure that all information and roster is up-to-date. Instructions to update burn center’s organization’s page.

Rescheduling a site visit will result in a $1000 rescheduling fee. |
Submit the application.
Click “Submit & Pay”; this is a free application; no fee is required at this time.
Once the application is approved by the Verification Coordinator, you will receive a notification that your burn center’s Pre-Review Questionaire Questionnaire (PRQ) is ready.
