The Summary Report and PRQ are uploaded to the discussion thread for the burn center that is up for vote and the summary report is sent to the burn center along with determination letter.
Follow steps below to download summary report and PRQ.
After signing into BCQP, click the verification tab and search for the burn center you need to save/print. When you are on the burn center’s page you are looking for, click on Site Report.

The first page of the site report will pop up which indicates it is now downloaded on your computer. Go to you downloads and “save as” to the burn center’s file located in the verification file in the shared folder as [NAME]Burn Center_Summary Report_YEAR.

Go back to the burn center’s main page and click on the PRQ and download load it and then “save as” to the same folder as PRQ_[NAME] Burn Center_YEAR.
