You can also find the application when are on your organization’s page in the panel to the left. Scroll down to Verification and click the arrow. Click on Verification Application. |

Go to Organizations on your page and click on Manage.

Go to Links and clickon MANAGE YOUR INDIVIDUALS.

Once on “Certification Application Forms” click on Site Visit Verification Application, then Complete Application Form - click on Start Application.

Enter information into blank fields on application.

Select Credit Card or Bill Meand complete the required information.
Then click Submit Your Order.
Once the application is approved by the Verification Coordinator, you will receive a notification that your burn center’s Pre-Review Questionnaire (PRQ) is ready.
