User must be set up to purchase on behalf of the organization. To check, you go to My Profile and click on Relationships. Under the “Organizations” tab. It must show on this screen that you are the “Account Administrator”. If not, email verification@ameriburn.org to request access.

If you are the “Account Administrator”, go Go to Organizations on your page and click on Manage.
Go to Links and clickon MANAGE YOUR INDIVIDUALS.

Enter information into blank fields on application.
Click the arrow and choose Adult, Pediatric or Adult & Pediatric. Provide preferred dates for the site visit that are within two months of the verification expiration date and provide any blackout dates during that time period.
