Verification Application Process
The burn center’s Account Administrator is the only individual who can apply for verification. They must log into members.ameriburn.org to complete and submit the verification application.

Set-up and Information Confirmation
User must be set up to purchase on behalf of the organization. To check, you go to My Profile and click on Relationships. Under the “Organizations” tab. It must show on this screen that you are the “Account Administrator”. If not, email verification@ameriburn.org to request access.
Review and confirm your organization’s record is correct including Name, address, and phone number. If anything is incorrect, contact your organizations ABA Account Administrator abacentraloffice@ameriburn.org.

If you are the “Account Administrator”, go to Organizations on your page and click on Manage.
Go to Links and clickon MANAGE YOUR INDIVIDUALS.

To add BCQP Verification Users, scroll down until you see BCQP Users and add individuals that will need access to the PRQ. Please note that they must have a profile on the ABA website. Please instruct individuals that need to create a profile to go to Member-Services on the ABA website.

Completing the Application
Before you start the application, you will need to upload an Application Signature Page. A sample is available to download.
Under Certifications, Click Verification and Click “Submit Application”
Choose “Purchase for X Organization”
Once on “Certification Application Forms” click on Site Visit Verification Application then Complete Application Form.
