Applying for Verification
Enter burn center name into search box and click “next”. Click “start application renewal”.

Please ensure that all information and roster is up-to-date.
The signature page must be signed and uploaded before the application can be submitted and approved.
Burn centers should ensure that the provide preferred dates for the site visit that are within two months of the verification expiration . Also ensure that the blackout dates are correctdate and provide any blackout dates during that time period.
