If there the recommendation is to defer verification and/or if there are comments, the Verification Specialist Coordinator composes the determination letter using this template.
When comments are provided in the discussion section, the Verification Specialist Coordinator edits and inserts the comments into the determination letter. The comments should be edited to be grammatically correct and formal.
For deferred determination letters or otherwise complicated determinations, the letter is to be sent to the Executive Director VRC for edits, and then to the VRC Chair.
Once approved, the letter and site report is to be sent to the BC Medical Director and Verification Coordinator. CC the site reviewers, Executive Director, and the VRC Chair, Verification email, and Verification Specialist.
